How To Schedule a New Patient Appointment

If you are new to Fox Chase, you can request an appointment by:

Fox Chase Cancer Center accepts both self- and physician-referred patients. Please be prepared to supply the following information when you request a new patient appointment:

  • Your name, address, telephone number and date of birth
  • Diagnosis and date of diagnosis
  • How you received your diagnosis (i.e. CT scan, biopsy, etc.) and at what hospital or facility
  • Name and contact information for your referring physician and/or primary-care physician (if you would like Fox Chase to send reports to your physician[s])

Once you have provided us your information, we will work with you and a Fox Chase clinician on a review of your records. This review will assist us in determining which of our providers and/or clinical programs you should be seen by. If you have a referral to a specific provider, we will work with that provider on the clinical review.

Scheduling Your Appointment

A New Patient representative and/or a Nurse Navigator will match you with a Fox Chase oncology specialist and help you schedule your first appointment as quickly as possible.

Pre-Registration

Once you have finished with the New Patient representative and/or Nurse Navigator, you will be asked to speak with a registration staff member who will collect your insurance information, including the name of your insurance company, group number and ID number, and for those with an HMO, your physician provider number. (If you have already submitted this information online, it will be reviewed for accuracy.)

If you have questions before your first appointment, need to confirm that your medical records were received from another medical center, or if you need to change your pre-registration information, please call 215-728-2570

Shortly after scheduling your appointment, you will receive in the mail a Fox Chase Cancer Center's New Patient Packet, which will contain important information about your scheduled visit(s).

Automated Appointment Reminder Service

A few days before your scheduled visit, our automated phone system will call to remind you of the date, time and location of your appointment at Fox Chase. In the event your appointment must be canceled or rescheduled, we will mail you a letter or call you directly with new appointment information.
