What is Care Connect?
Care Connect is a collaboration linking Fox Chase Cancer Center and community physicians. Primary care and internal medicine physicians in communities throughout the Philadelphia region are working with Fox Chase in areas that support the integration of cancer prevention education, screening, treatment and survivorship care plans into clinical practice. The result is a seamless coordination of care for patients.
The Care Connect Distinction
Fox Chase has earned the highest designation from the National Cancer Institute (NCI) as a Comprehensive Cancer Center. These elite centers are recognized for excellence in cancer treatment, research, prevention and education. We are extremely proud of this achievement, and are careful in selecting Care Connect members only in physician practices that share our commitment to the highest standards of cancer care.
