Care Connect
Care Connect is a program between Fox Chase Cancer Center and primary care/internal medicine physicians in communities throughout the region.
The Care Connect Program offers patients easy access to Care Connect physicians who have knowledge in the latest developments of cancer care.
We offer support for primary care physicians in the areas of cancer prevention education, screening, treatment, and survivorship care plans. The result is a seamless coordination of care for patients.
